Create a New Account (After PIN is obtained)

In order to make full use of our online services, you will need to create an account and verify that you are registered with the clinic. Please use the form below to create your account. Once your account is created, you will proceed to the verification process.

Please note:

(1) Your User ID does not have to be the same as your email address.

(2) Email addresses @TuftsMedicalCenter.org cannot be used for patient access. 

(3) The email address provided below must be unique; one email address can only link to one patient. If the system recognizes the email, you will receive a red error message. This means that the email was used already for another patient (often husband and wife sharing an email) or that your account was setup (even though it may not be linked to your patient record yet.)
